Flinders University PhD candidate Sharmin Sultana believes AI chatbots could transform mental health access for 1.1 billion people worldwide—but only through ethical, inclusive design that puts human care first. Here she speaks on what made her curious about AI Chatbots and why she commenced a PhD in this topic.
“Mental health needs are growing faster than health systems can respond. Artificial Intelligence (AI) is emerging as one way to widen access – if we design and govern these tools responsibly and keep human care at the centre.” she commented in her
recent opinion piece in Health Leadership Insights.
Drawing from clinical work across Bangladesh, Thailand, Japan, and Australia, she highlights how back pain often masks untreated anxiety, migrant needs vanish from data, and stigma silences even Japan’s kodokushi crisis. AI offers 24/7 breathing exercises and check-ins for underserved groups—older adults, rural communities, migrants—but demands multilingual interfaces, voice input, large text, and clear human handovers.
“The main barrier now is not technical capability; it is leadership capability and policies we make. Health services need implementation models and well thought policies and frameworks that are safe, transparent and accountable from day one.”
Sultana warns of hallucinated advice, privacy breaches, and designs that widen inequities, urging leaders to ask: Who validated this for my population? What happens in crisis? “For a Burmese worker in a Thai border town, a chatbot that speaks their language and is available on a smart phone could be the first mental health resource they have ever had access to.”
