WHS – reporting an accident, incident or hazard

Accident/ incident, near miss and hazard reporting is a key component of work health and safety. This reporting allows the University to identify hazards, manage safety risks, identify trends and respond to and support those who have had an injury. The aim is to continually improve the health and safety outcomes for all of the University community.

Reporting

All accidents/incidents or near misses need to be reporte. Definitions as what constitutes an accident/incident, near miss or hazard can be found here.

In the first instance, it’s important that the area manager/supervisor or other relevant University staff member is notified by telephone or email as soon as is reasonably practicable. This is to ensure that any immediate corrective actions have been taken to make the situation safe and prevent anyone else getting hurt. If a person has sustained an injury, it is important that they get the immediate help they need, be that emergency services, first aid or medical.

Reporting is via FlinSafe and must occur as soon as reasonably practicable or within 24 hours of the occurrence. Failure to report in the required timeframe may put others at risk and potentially affects the required investigation and actions that may be needed to support the injured person.

Access to FlinSafe is via Okta and can be used by all staff, students, contractors, and visitors.

Notifiable Incidents

In certain circumstances where there is a serious accident/ incident or near miss it may be classified as a notifiable incident . This includes a death of a person, a serious injury or illness or a dangerous incident. These incidents must be notified immediately to University Security or the WHS Unit.
